If i understood it correctly. All canthos are designed to fail unless it’s specialized. The regular cantho causes a host of problems from rounding which will happen because of the high risk of skin retraction/ lid canthal retraction. The retraction occurs because of the muscle/ skin/ fat of the lower lid/ upper lid is taken out, damaging the musculature of the eye.
The fat and muscles help keep the canthus sharp. Skin retraction occurs because there isn't enough canthal skin and canthus retraction occurs when the tendon is cut or severed, the lateral canthus shortens which sometimes can cause the sutures to pop out.
The only alternative is drill bone cantho. Using permenent sutures and then taking care of your eye and doing the proper post op care to make sure retraction doesn’t occur
