I think I get what you're saying. His cheeks have a bloated look, as if he has smth in his mouth, despite being low BF it's over Maybe low-set cheekbones.

Yeah people sometimes underestimate the importance of lips and nose when it comes to influencing how your other features look. His lower lip does seem a lot more protrusive than his upper lip, but not in an underbite-looking way. His upper lip might be turned inwards making it look smaller, idk.
