Partly because they don't know about Bimax and partly because even if they did they would reject the idea of spending 2 years in braces for an invasive surgery with a huge recovery time that costs a fortune.

I think it's a risk/cost/reward kind of thing. Obviously CCW rotation super bimax with genio gives you a better result than a chin implant. But there are deminishing returns on investment.
