CanthoPEXY is less invasive, it's also more effective, canthoplasty is botchable. And fails within the years. PEXY is also cheaper.

I understand canthopexy as a canthoplasty's younger cousin, definitely cheaper and less invasive but also you wont see as dramatic of results with just a simple canthopexy imo. I feel like it really depends on how severe your case is if anything. Canthoplasty might be higher risk but high risk = high reward.
