I have almost no muscle tone despite working out four times a week, I have 750 NG test on my blood test, so thats not something to worry about, yet my body looks
like this. Is there any way to fix this? I am very underweight so its not fat distribution

What do you mean? You Have literally nothing except bones, skin and organs. Go to gym

ive been eating around 4000 calories everyday for the past 2 weeks yet im still very skinny, I dont have an ED, I am naturally really skinny despite eating the same amount as everyone else and my BMI is around 16

Nonsense. When skinny and a hard weight gainer you need to pound calories to gain any weight in addition to lifting.
 
On a real note you don't know how to track if you say your eating 4000 calories and looking like that, that's not even my total daily expenditure at 6'6 with muscle. The problem is almost certainly with your nutrition.

I have an obsession with trying to eat the healthiest I possibly can, I think I have a good diet
 
I mean, you gave yourself the answer.. you're underweight. Working out (and it also depends what kind of workout, is it weights, cardio, a mix of both) doesn't guarantee a muscular physique, it's the most important part, but you also need to factor in your diet, recovery and if you're training correctly in the first place. Here's the most solid advice I can give you as someone who went from 141lbs to currently 183lbs.

- Buy a food scale, download the Cronometer app (MyFitnessPal is bad), track your calories and weight. Make sure you're in a caloric surplus of 250 calories at the very least. Bonus points if you just create a diet regimen and eat the same foods most days so you know for sure you're consistently gaining weight and don't have to worry about some days eating low calorie foods, and others food that are way too high in calories. I ate pretty much the same 4 meals every day for like 12 months, of course occasional missses because of life events and such, but I knew how many calories I was eating, I knew exactly what I need to buy from the grocery store and doing caloric adjustments was pretty issue, plus I could also easily track my micronutrient intake. Try not eat too much protein or fats as they're too filling, 20-25% protein, 20-35% fat, the rest are carbohydrates. On a 250 calorie surplus you'll be gaining about 0.5lbs per week, which is good for mostly gaining muscle and not too much fat.

- Make sure you're following a proper workout routine, most beginners (you're a beginner if you have less than 2 years of working out, and can't bench 2 plates, squat 3 and deadlift 4 plates, period) have no idea about proper programming for their training experience, no idea of exercise selection or how much volume is too much. So instead of actually having to learn that, look for a workout routine made by somebody else who actually knows, and stick to it until your progress stalls. There are tone of resources online, and even an app called "Boostcamp" I recommend with a huge variety of workout routines by famous content creators, and bodybuilders for all kind of goals and experience levels. Do mostly weight lifting, and minimize cardiovascular training, if you do want to do it, make sure it's mostly zone 2 cardio, keep HIIT or other harder forms of cardio to a minimum as they can tank your ability to recover.

- Sleep, get sunlight, manage stress, standard stuff, but yet fundamental. I assume you're good in this department, since your testosterone is slightly above average.

While its good to eat healthy, it's not the be all end all. For foods on the healthier end to gain weight, use a fattier cut of steak, use more butter in cooking, or drink whole milk. Peanut butter is starting to edge towards the junky food but is still good for you.

I’m pretty sure he’s not really hitting 4000 calories, probably miscalculating. Unless he’s super tall, in which case it could make sense, but even then, he’d need to have a ton of muscle mass or be super active. But honestly, no one can break the laws of thermodynamics. If he needs 4000 calories to maintain his weight, he just needs to eat a little more than that to start gaining. The body will store any energy (calories) above maintenance you take in, that’s how metabolism works. So, really, it just comes down to eating more until he starts seeing results.

Issue is most people simply aren't in a deficit, or think they are but just don't know how to properly track calories, either they overestimate how much they eat, or they underestimate how much their body naturally consumes.

I knew people that ate 5k. If the person started gaining sloppy weight they could just lower it down if needed.

Some people as teenagers just won't gain weight hardly and need to go really high and lift to force some on.

Lift heavy and close to failure with good form, do enough sets but not too much, stay less than 1h and a half on the gym

Its not the frequency nor the volume nor the exercises, its the quality of each set, each rep
