Drs. (as anyone) think in the best of them, not the best of the patient, and the best for them is that 100% patients get satisfied with the results, which is different than providing the best results
And why this is relevant here? Because if they try to go for the best movement they will risk to go beyond it, and patient think it was worse than before.
Just to provide an example. If the best movement is advancing around 80% something that is recessed, the best thing would be try to advance it, however will risk to have become more advanced than normal, and patient might not like it, as he was not used to this failo before´
But if dr. advance only 50%, it will certainly be better than before, and patient was already used to that failo (recession), however now will be less noticeable
So what you should do?
1- Don't go to bluepilled drs. they are much more concerned to just improve, than to get the aesthetically ideal
2- Be willing to expend more to go to better Drs. and have better results, surgery is not something you want to save money
3- Try to have simulations before and tell him what you want, insist a bit to be a bigger movement if you see that the Dr. is being too conservative
