Most class 2 and class 3 patent have disgusting ramus and gonial angle
Why posterior downgraft and ramus lengthening to flatten occlusal płatne are not so popular. Plus there should be already better ways than surgery\or surgeries who have more options/ and/or are less risky. Actually because of pandemic some say that medicine rewind like 25 years which is not good.

Actually ramus IS more important for Airways opening than lenght of jaw body.
I hope tempomandibular joint is not great limitation?

Yes but they could also move ramus down and lenghten it
Not only for aesthetic reason (short ramus =shitty motion Look) but also problem with mouthbreathing even if you try nosebreath, weak masseters etc.

Yes, man my gonial angle needs to be lowered at least 10 degrees
 
Vaughan told me it's possible to lengthen the ramus with distraction osteogenesis but he doesn't usually recommend it.

tldr?

I have yet to see a surgery that improves ramus+gonial angle other than a jaw implant (which look terrible).

I guess because it has nothing to do with the procedure. A long chin wing should work for this, lengthens the ramus without touching the bite so surgeons wouldn't be too hesitant to perform it.

massetter function & stretching of the ligaments is a big part of why vertical ramus lengthening is such an unstable thing to acheive and comes with a big chance of relapse

iirc there's now a work around for alot of the relapse risk but, idk what we can do about this really, even if you need ramus lengthening you won't be able to get it for cosmetic reasons

Do you know if it's possible to do the long chin wing (I am assuming this is side wing that widens at the gonions) after you healed from a regular Bimax?

Good to know. I was recommended Trimax with CCW. Just wondering if I should do the side wing first for width at the gonion, heal from that and then do the larger surgery as noted prior.

I didn't ask him in detail. he told me in my case simply BSSO would be sufficient. He also offered me distraction osteogenesis as an alternative to Lefort 1 and BSSO but says he's only done it before once on a child.

I asked him if distraction osteogenesis lowers the risk for nerve damage, and he said that in theory it would, but he doesn't have enough experience with it to give a definitive answer.

Its called IMDO it legthens the ramus and grows that area three dimensionally unlike a bimax.

ImDO for sure will give taller lower third but ramus Lenghtening would only give square mandible

CCW Bimax surgery plus chin wing should fix the failo even for overbite cels. If gonial is still too high neckmaxx and grow a beard that's all you can do
