Why go intra-oral instead of through the eyelid for cheekbones and infraorbital implants?

Pretty much all the muscles and meat is ripped right off the bone and the huge ass implant is squeezed through the mouth. How can there be no risk of damage with this approach? And they're putting it in nearly blind, which makes a saddle style IO seem impossible to get right. With the eyelid approach everything is visible and not much tissue is being pulled off
