Because most people prefer being lean or getting an implant instead of just removing the fat. I suppose you can do all 3 if your choices were limitless, it's just not preferred and people don't have unlimited money (and would rather spend on some other cosmetic surgery).

buccal pad is tiny and makes almost no difference, it won't age you because you literally will not be able to tell the difference.

if you're a good candidate you could. but the results may be underwhelming and not worth the money.

for example this girl on reddit is 115lbs and still has a chubby baby face. she paid 5k~ and still has chubby cheeks after the surgery, but there is improvement nonetheless.
