balding is a side effect that was never bred out of the population because there was no need for it, it's probably happening now though, since bald men don't have much reproductive success due to people settling at a later age now.

it depends on your norwood type, the best responders are diffused thinners, once the hairs are gone and scar tissue has started forming you're basically fucked.
