idk man, the arch extensions will make better results for the hollow cheeks also combining it with wide gonials will make huge improvement thats why i wanna do it but the upper third will be the issue.

And for the esr i think the extensions gonna affect it negatively only if the esr is low cuz the extension will make it appear even norrower and closer right?

The arch extensions don't matter for hollow cheeks. If you already have a 0.44 esr for example, arch extensions will lower the esr even further, although minimally.

I was talking about the upper third.
But for the lower third i think it's more ideal for the cheekbones to be approx the same width as the gonials like 1:1 ratio
