Accutane dosing high IQ query

I will be taking accutane 20Mg for about 13 months to reach the cumulative dose.

But there is other option of cutting it down to 6 months by increasing it to 40Mg day

Just one query is there any downside of the first option is any option superio here in terms of permanent affectivenss? Or second is superior?

Time is not an issue for me since I have patientcell
