tret or taz overall? My skin is fine no pores really just dont have that super glowy look yet. Microdosing isotret 10mg a day rn should i stop it or just add tret or taz?
You could try adding tret or taz, I'm personally going the taz route. Tret is super "normified" and is overly used when taz exists. I'm not saying it's niche but it's more niche than tret because of normies. I could see differences literally in a day.

Add taz and if it made a big difference to your skin, that shows you that the accutane is doing nothing. You can do a little study on yourself like i did, I didn't have the glowing until adding taz hence I know accutane even at 40mg can't compare. For MY skin at least.

This idea that you should just "microdose accutane" for skin maintenance is so retarded and is a pseudoscience Clav has pushed out arbitrarily. Accutane does not penetrate the epidermal or Stratum Corneum layers of the skin, which is literally what we all see and feel on our face. This alone is already why topical retinoids are better.

Studies also have shown that Topical retinoids like fucking tretinoin already beat out low dose accutane in photoaging, wrinkles and pigmented skin. Which is what matters for maintaining how good your skin looks mainly. (With some other factors I will address)

Topical retinoids being applied directly onto the skin give it the advantage of changing the SURFACE of the skin. Accutanes main mechanism of action is the atrophy of Sebaceous glands (Located in the dermal layer), but being a retinoid it also does the other things the other retinoids do at a systemic level.

This is why it's also less effective for the skin because it's a spread out effect that comes from deep within the skins layers contrary to being localised onto the skin surface.

This is also why accutane can't get rid of Post-Inflammatory-Hyperpigmentation or Post-Inflammatory-Erythema, I like to think of Accutane simply as the ACNE compound not a skin compound all in all.

Skin cell turnover rate is another factor that matters but follows the prior principle of it being systemic vs topical. It sounds like an echo chamber but it matters a lot, the retinoids practically all have the same functions with accutane being a systemic version that affects the skin more deeper. But the localised action of topicals beat accutane out for skin APPERANCE.


So Kojo, if this is all true, what do you recommend I do for my skin?

Honestly I won't really overcomplicate it, You just need to attack your skin failos. Assess and diagnose the problem, look for what fixes it.
PIH/PIE - Hydroquinone for 3 months, cycle off into Azelaic Acid to maintain your skin from the pigmentation. (for pie use azelaic acid too) Use a retinoid to actually permanently shed that outer layer of your skin that's pigmented.

Azelaic Acid and Hydroquinone are tyrosinase inhibitors so they aren't permanent they just inhibit the melanin on your skin whilst your retinoid will actually shed the skin permanently. It should be permanently fixed within 3-6 months depending on severity.

Acne - Full accutane course, meet a cumulative dose of 150mg * your bodyweight in kg. You will typically get rid of pih/pie after doing this course but if you're giga low inhib you can do it whilst on the course like me. it's burning the shit out of my skin but who cares.

Skin Texture/Glow/Collagen - Are mainly stimulated by skin cell turnover rate and exfoliation (removing dead skin cells from the skin) in which Topical Retinoids do for you and directly on your skin, alongside chemical exfoliants.

Pairing this with something like glycolic acid can help chemically exfoliate your face, I'd only do this once a week though to avoid over-exfoliation. But imo topical retinoids and a full accutane course will mostly do the job for glowing skin and a good skin texture. (I'm seeing results in 3 days on tazarotene 0.05%)

HOW WE WILL MAINTAIN OUR RESULTS.

Tazarotene/Tretinoin - Every single Night
Cleanser/Moisturiser - Every morning and Night (Water)
Chemical Exfoliants - Once a week (preferably glycolic acid after skin gets used to retinoid)
Sunscreen - For photoaging, Use whenever going out, especially on a sunny day

Just be hygienic in general, Drink a lot of water and change bed sheets weekly. Wash your hair too.

Keep. It. Simple.

when should i introduce my glycolic back into my routine? been on epiduo for more than a week now and i still get occasional breakouts ( retinoid purge + bad genetics )
i still have blackheads and bad skin texture.
is patience all i need?

Patience is a very big factor, it'll take you a month at the bare minimum to see even a significant change to your skin with anything here.

I personally wouldn't use the benzoyl peroxide, pretty useless when you have accutane to solve the acne issue. Just let the acne run it's course, whilst accutane runs it's course. It'll be over soon, don't bother trying to obsessively fix your acne break-outs.

Adapalene isn't too bad, if using it for breakouts don't bother just use tretinoin to at least help your skin quality and appearance.

I wouldn't exfoliate on accutane personally whilst also using these other compounds, You'd just be RAPING your skin barrier. You'll likely get red from it too (Happened to me)

TL;DR Let accutane run it's course, forget the other compounds. If you can handle it then use tretinoin or taz. Do NOT exfoliate.

Edit: I didn't read the azelaic acid part, Azelaic is good for managing inflammation/redness and Hyperpigmentation. I'd stay on that for sure.

I was on tretinoin (tret+clindamycin in one) but it was way too harsh to use on 60 mg accutane.

The adapalene im on has benzoyl in it so i dont really have a choice when it comes to that. But atleast it dries out any breakout the next day so...nice benefit i guess.
Im using the adap for skin texture as i've read its "gentler" that the other retinoids while still being effective for texture.

Hey buddy have you ever thought that we take these things for acne rather than photoaging and wrinkles? Anytime I see someone say topic retinoids are better I just know they are older than 30. Accutane is the only thing that will permanently remove your acne, not tretinoin. I will go ahead and agree with you on your points but you are completely missing out on the fact that people aren't taking accutane for anti aging lol

You're also missing out on the fact that I literally did not say topicals serve as a functional equivalent or substitute for accutane in resolving acne.

And the point is that people are starting to use accutane for aging and skin quality when it's shit for that.

Read the studies, tret gives the same results on the long term regardless of the concentration.

I used them for years.

I haven’t tried Taz though, but I still don’t care that much about retinoids. There’s better things, retinoids are basic and not that good.

If you want good skin you spend the money on treatments.

Treatments are always going to beat any appliance. But that's not as cost effective for a good majority. And I'd say you're on the minority side for not getting much results from retinoids. Could be more factors at play here.

Send studies on concentrations not having a difference. Also adapalene is more acne-fixing than skin quality based.

Yes, I suppose diet can play a role in reducing inflammatory responses but it's majorly cope. A lot of people with a consistent acne pattern have hormones as the cause. Not food

i mean like, its for real.. i tried dieting so hard like fuck im not eating some dopamine donut shits and still get acne.. but now i eat like shit (bulking for gains anyways) while using a retinoid which is tret and i dont really breakout.. purging yes but breakout really no

I agree and i was planning on making a post just about this, idk why retards keep saying Accutane to shit like pie and pih it doesn't shit and tret mogs it for skin quality, BUT i think you're underestimating the percentage of people whose glands relapse to high oil production again, they're not that small and micro dosing Accutane afterwards makes sense for them. Also Accutane can be good for oily people with acne as well. But good post bhai

I agree with you and I'm not coming at you, but just like 90% of other people that read this post you're also miscomprehending the point. Yes accutane is an acne remedy, I'm not underestimating oily skin rebound/acne rebound that simply means use accutane again or perma dose.

But that's for a different cause, not anti aging or skin quality.

Topicals would already solve an issue like this.

Salicylic acid? It's good for oil related failos like sebaceous fillments or pore decrease, but i was referencing that greasy skin look oily people get. What topical would stop that. Decreasing oil production is ideal for that.

Retinoids, they don't exactly decrease oil production or affect deep layers of the skin but they do dry out the upper layers of the skin which reduces that oily look. Gives you the smooth matte look you're going for. They also unclog and decrease the appearance of pores.

Edit: And yes salicylic would be good too, any kind exfoliation for that fact.
