Accutane not working !

I’ve had acne since I was around 14. Back then, my dermatologist prescribed me 10mg of isotretinoin daily, but I didn’t follow through consistently and instead tried to manage it with skincare products over the years.


As I got older, my acne improved compared to my teenage years, but it never fully went away. I’m 20 now and still get around 3–5 cystic pimples every month. There’s basically never a day where my skin is completely clear — I almost always have at least 1–2 active pimples.


Because of this, I decided to start isotretinoin seriously about 3 months ago. My dosing so far has been:


  • 10mg/day for 3 weeks
  • 20mg/day for 3 weeks
  • 40mg/day for the past 3 weeks

Even now, I’m still getting acne.


I make sure to take it properly with high-fat meals (usually at least 12g+ fat), and I split the dose into 20mg with lunch and 20mg with dinner.


What could be the reason I’m still breaking out, and what would you recommend doing from here?
 
Just do 20mg for 2 years and you will have clear skin.

bruhh my acne was so bad like u cant even look at my face . it was disgusting . changed 3-4 derma to get good results
 
Just go to a derm boyo
i already did 4 days ago . he said my skin is fine and i need 8 sessions of co2 laser and some combo sessions of micro needling and peels . i don't know if he was being honest or just tryna sell procedures .

You can do microneedling at home with a stamp/roller as long as it’s sterile but do not do it for acne that’s active only acne scars.

Yea if he did recommend that he’s defo shilling shit

You have to do 0.5mg per kg of weight.
Never skip a day and continue for at least 6 months.

I’m not planning to do all the scar sessions my derma suggested. Thinking of doing 1 CO2 session, then 1 CO2 + subcision combo since I have deep tethered scars. After that I will probably continue with microneedling and TCA peels at home. Does this sound like a good plan ?

is there anything i can do to speed up the process? also should i get laser resurfacing and subcision procedurs in this 6 months ? my derma said its alright we can start

you need to be very careful with procedures such as laser resurfacing and subcision, because the skin is more fragile, slow to heal and more prone to scarring or pigmentation changes.
During the treatment you can take vitamin C but nothing else because the skin is very sensitive.

If you have severe active acne absolutely do not do a CO2 session it’ll just irritate you more.

If you have scars or scars + small amount of acne they probably can do that
