2- human ugly due to health problems, like very old, or missing teeth

Your too lenient. The guy has a recessed jaw, crooked nose, high hairline, bad skin, poor eye area, he's fat and old. That's like 2-3/10. He's an incel. 4/10 is LTN - just a below average guy.
Although there's no effective difference since both are sub5.

No he's not, he's like a 3, if he was bald and uglier a 2. Average person is a 5, he can't be a 4
