they create an incision from the inside of the lips and cut a thin line out, and stitch.

You probably don’t need it and it’s just other parts of your face that are recessed

Also, the lips shrink as you age, you don’t want ridiculously thin lips later in life

My lower lip seems to be to big because lack of mandibular support. Maybe you are right.
