I don't think its the nose thats the problem its my small lips and ipd that gives the illusion my nose is out of placeLip filler and rhino. Maybe some more nose bridge projection, narrowing the nose would emphasise the higher set ipd.
Heres my eyeball with camera flash on and my jawIt's unfortunate that both of your major flaws are nearly impossible to fix. Especially as your bone structure and definition are top-tier.

As for commissuroplasty, unless we (collectively) have missed out on some published data, it doesn't deliver significant cosmetic improvement and comes with its share of potential drawbacks (visible scarring, unnatural look).
At this point, if we're being reasonable, the most you can do is lip filler, as suggested earlier. And, of course, all the relevant softmaxxes : tretinoin/tazarotene for your skin, minoxidil + microneedling for your eyebrows, etc. ; I'm usually not a proponent of colored contact lenses, but it might make a nice difference in your case (if the picture displays your natural eye color accurately).
Also, based on the shadows casted below your mouth, it looks like you have some sort of protrusion (underbite ?). A side profile picture would help to further assess this point. If you're not already seeing an orthodontist, you might have to think about it.
