is it just too much protrusion or what
does his palate contribute to it since it looks somewhat narrow in relation to his total face width

because i also have that flaw but worse since im not lean and have no infras there's less bone support for the fat pads so my midface always looks bloated and heavy/droopy

No structural issue whatsoever. Could be due to many factors, like very pronounced cheeckbones thus more prone to crease when speaking, smirking, etc... Prolly didnt use any type of sunscreen or skin treatment and so on

Mainly its because his bizygomatic width is so high but his palate is narrow so its not proportional with midfacial span so the soft tissue is forced to get fucked
