cutting the medial canthus wont convert a neutral tilt into positive tilt youd mainly be altering the shape/position of the inner canthus, not actually elevating it relative to the lateral canthus

if ur trying to improve a neutral tilt the relevant thing to look at is the actual lateral canthus position and surrounding orbital/eyelid anatomy
