CBCT what can I do?

Hi,
I'm 32 year old and probably been a mouth breather all of my life until I realized it wasn't right. My nose was always clogged and I thought it was normal. As a kid I had a really wide palette and jaw and surprisingly good projection as well. Over the years my face thinned out, my profile changed and you know how that shit goes. Anyway, my jaw from the front is not narrow, but the issue is the 3/4 and the profile for me, mostly.

My father has a really well developed skull with huge jaw.. similar to Brad Pitt.
All of his family does. So I know it's not genetics.

Anyways, my issues are functional and aesthetical. My narrow palette causes me issues with breathing and I have bad sleep quality. I also have issues with neck posture which I believe the jaw contributes to. I also suffer from TMJ as a result. Feels like one side is shorter and as a result of the jaw structure/neck position the TMJ gets jammed against the glenoid fossa when I jaw or even rest because I feel like my jaw is retracted. I found a direct relation between the jaw and the neck. There is this habitual thing going on where I automatically protract my jaw forwards in order to breathe better and be in a better position, but it causes other issues..

I don't even know if my case warrants bimax or whatnot and since I have some medical issues that may be against this kind of surgery, i'm being cautious.

Get a sleep study done. You likely have sleep apnea and with this jaw surgery becomes medically necessary and covered by most insurance

in the meantime try breath right strips to see if that opens your nose and helps you sleep better

how the fuck did you get a CBCT scan? its fucking near impossible for me.

I either need a referral or the clinic doesn't do it

I had it done for the TMJ. I insisted.

I had a sleep study done.. couldn't fucking get to sleep. I guess I will have to try it again, though I would prefer to do it at home this time. Do you see anything in the scans that may lead you to think that I have sleep apnea?

i dont know how to read an xray, and nobody here does, only an actual oral surgeon will help you. All I can really see is a mild overbite, but your symptoms are enough to suggest you have sleep apnea, which is often accompanied by recession therefore making you bimax candidate

just keep trying to do the study until you sleep. you can do an at home one. are you in the USA? Try Lofta

Same I can’t find a clinic that does what my referral requires

Your occlusion is fine.
Your occlusal plane is a bit down swung so you could benefit from CCW.
Don’t know about projection without seeing soft tissues really.

A “functional bimax” on insurance just advancing both jaws equally would probably be adequate unless you’re really unhappy with your Midface length.
 
Can’t say. Ik there’s a clinic that will be guaranteed do it bc they specify they do CT scans for jaw surgeries but it’s 2 hours away from where I live

at least in usa orthodontists usually do free consults where they take cbcts and you can request these on a file after

you cant tell shit with CBCT

Send profile pic minimum and XRAY if you have

Soft tissu is more important than ur CBCT to see what u need
