1. depends on your occlusion, only if you already have normal occlusion or you're advancing the maxilla beyond what is needed to maintain proper occlusion

I guess expansion is limited by increasing infection rates and soft tissue rather than inability to expand the bone itself.
The surgeons you tagged will obviously know more.
But why?

1. Yes if for aesthetic reasons/already in good bite then the lower jaw must be advanced

2. Yes and you can also have a combined LeFort III/I

3. Apparently Douglas Sinn does a similar movement called a tripod osteotomy

4. It cuts the orbital floor but it doesn’t break it up if done correctly. Large gaps will require orbital floor bone graft or titanium/silicone/medpor orbital floor implant which is how orbital floor fractures are corrected
