Explain why

His mouth is more upturned and his mouth shape is changed. For his jaw, he'd have to get wrap around implants which is at high risk for infection.

His orbitals are almost completely changed so that's orbital box osteotomy which is extremely high in risk factor.

His infraorbitals are much better so that's infra implants which are unreliable. The entire eyebrow shape and height has changed.

The only truly realistic thing here is otoplasty with eyebrow darkening.

I looked at it again and we're both wrong. His skull entirely got wider (slight scaphocephaly with possible temporal bossing), and the zygos higher set. His orbitals got more supported so that's a bone thing and the results were ameliorated with the upper blepharoplasty and canthopexy.
