1. Introduction
I'm pretty sure a lot of you already faced multiple times not being hungry while still wanna build muscles. People tend to tell skinny guys "just eat more" "muh calories bro, just eat 5000 kcal"

Some people genuinely have trash appetite. You can tell them "just eat more" 100 times, it doesn't change the fact that forcing 3000+ kcal every day is miserable While a calorie surplus sounds easy in theory, it's harder in practice.

A perfect program doesn't matter if you can't consistently hit your calorie target.
That's why boyos, in this thread, I will talk about a drug : Cyproheptadine, that can massively help some people with low appetite, your lanklet body to feel that hunger and finally be able to eat enough :Comfy:.

2. What is cyproheptadine?
Cyproheptadine, sold under the brand name of Periactin as a hydrochloride salt, is a first-generation antihistamine, which means its original purpose is to treat allergies.

However, doctors quickly noticed something interesting: people taking it often experienced increased appetite and weight gain. Because of this, it has also been used in underweight patients and people struggling with poor appetite.

3. Mechanism of action
Now for the autists who care about the actual pharmacology,
The appetite effect of cypro doesn't come from its antihistamine action alone. It mainly comes from blocking two types of receptors:

1. Histamine H1 antagonism
Histamine isn't only involved in allergies. It also plays a role in the brain, including wakefulness and appetite regulation.
By blocking H1 receptors, cypro can increase hunger signals and make food more appealing.
This is also why a lot of first-generation antihistamines are known for causing weight gain. (Like lean aka Promethazine)

2. Serotonin 5-HT2 antagonism
The serotonin side is probably the reason cypro gets talked about so much for appetite.
Serotonin is heavily involved in satiety (the feeling of being full).

Cypro blocks certain serotonin receptors, especially 5-HT2 receptors, which can reduce those "I'm full, stop eating" signals.
In short terms boyos :
Less satiety signaling => easier to eat more => easier to maintain a calorie surplus

If the only goal is:
"I need something that makes eating 3500 calories less miserable"
For that specific goal, cypro probably makes the most sense. It's literally an appetite stimulant with decades of human use behind it.
Now obviously everyone is gonna ask "why not MK?"

MK-677 is a different category. People often compare them because both can make you hungry, but MK isn't just an appetite drug. People mainly use MK because of the GH/IGF-1 stuff, not just because it makes them hungry.
The trade-off is that you are also dealing with things like water retention :kys: and potential effects on glucose metabolism.
GHRP-6 is similar to mk in a way

Mirtazapine can absolutely make people gain weight, but taking an antidepressant solely to bulk is a completely different decision, a retarded one.

5.Dosage and sides
Dosage is where people usually fuck up because they assume more = better. Medically, doses are usually in the 4–20 mg/day range depending on the situation, although appetite use is often lower, with many adults requiring around 12–16 mg/day depending on the indication.
For appetite stimulation, the main thing people have to manage is not the hunger effect.

As I said, cypro is a first-generation antihistamine, meaning drowsiness is one of the main reasons people stop taking it.

The goal isn't to become a fat food vacuum JFL. More isn't automatically better, especially when the main side effect is feeling like you got hit by a truck.

6.Studies / Evidence
Systematic Review (2019) :

This is probably the best overview of the current evidence. Researchers looked at 46 studies involving cyproheptadine as an appetite stimulant across 21 different patient populations.

Out of these 46 studies:

-39 reported significant weight gain
-32 were randomized controlled trials
-The most common side effect reported was mild to moderate sedation

Basically, the research says it works for gaining weight, but most studies were done on underweight/clinical populations, not gymcels trying to go from 65kg to 80kg.


Randomized controlled trial in adults (2021) :
This one is probably more relevant because it looked at adults with poor appetite.

The study included 375 adults who were randomly assigned to either cyproheptadine or placebo for 8 weeks.

Results:
-Cyproheptadine significantly improved appetite scores compared to placebo
-The cypro group also had increases in body weight and BMI
-The most common side effect was somnolence

The result was basically what you'd expect: cyproheptadine was an effective and generally well-tolerated appetite stimulant in adults with poor appetite. :p

7. Conclusion
Overall, is cyproheptadine actually worth looking into?

Cyproheptadine is not some secret anabolic, it won't increase testosterone, it won't magically transform your physique, and it won't replace proper training and nutrition.
The whole point of it is basically one thing:
Making it easier for people with low appetite to consistently eat enough calories.
And honestly, that's a bigger limitation than people realize. A lot of skinny guys don't fail their bulk because they have a bad program. They fail because they spend months trying to force-feed themselves, miss their calorie targets, and eventually give up.

The funny thing with cypro is that it's literally an old boring allergy medication, but somehow it's one of the few things that actually solves the "I can't fucking eat enough" problem.
