it depends on the type of surgery but even when you don't have scars it tends to become a dimple as you get older, dolph had gyno surgery when he was younger:

(he developed it during the scorpion movie) I think and now he has this.

there's like an obvious depression underneath the nipple.

tbh this is way much better than gyno because he can wear whatever he want and can swim in public
