im 5,11 120pounds just started 250 test and 4 ius should i increase it to 500 6 ius next week or wait till i am at an healty weight like 140 150 bc i am worried my heart wont handle it if i am too underweight

just sit on 250 test and 4 ius and slowly titrate upwards.
the amount of gear you can actually utilise is limited to how light you are, so there's no point pushing up ur doses yet
