Do you need all the money upfront to start the bimax process

Assuming insurance doesn’t take me (I’m in the US), generally are you expected to pay the full 25k upfront? Or can you at least start the orthodontics, scans, consults, etc, with just a couple thousand? I’m looking at going to one of the Italians right now and I only have like 2k saved so far

depends on a few things.

if you need to do ortho first then that will take 1-2 years and multiple thousand dollars. you can only find out if you need ortho by getting scans and consulting a surgeon

if you are a surgery first candidate, then you need to think about your surgeon's waitlist. if your surgeon is scheduling surgeries in 6 months time (someone like ramieri), then invest in a consult now. if he has availability in 2 months time, then you might be able to wait longer.

generally the full sum for the surgery is expected at least few weeks before the date of the surgery.

also the italians are overrated

I’m almost certain I will need ortho, I have class 2 with the teeth tipping forward so I think they have to be decompensated to get a good bsso movement. I guess my main goal is to start ortho asap because like you said that’s gonna take a while, then wageslave during that so I don’t have to wait forever to get the surgery. So you’re saying you can start ortho with just a few thousand? Also, I was thinking about ramieri, but I do think he is a little conservative, do you recommend anyone else in particular?
 
You do not need the money up front in Europe at least you need a deposit (few k) to get a surgery date (few months) and rest is paid at the time. Idk about USA surgeons.

One thing you need to know about bimax is that the BSSO movement is basically set by your teeth and thus your orthodontist. There is only one correct occlusion pattern and that is what your BSSO movement will be done to achieve. So even if you have a big overbite, you will get the movement you need to fix it and put you in class 1 bite regardless of conservative surgeons. You don’t get to arbitrarily select that component of your BSSO overall movement.

Where the conservatism comes into play is what you do with the jaws once the occlusion is fixed (they will then move together as one piece). So this could include advancement of both jaws, rotations, down grafts, impactions.

So look at your problem as two parts: part 1 is moving your mandible forward to fix the bite and part 2 is whatever cosmetic shit you want.

It doesn't mean there is no variability based on conservatism in play, it just means it's about how conservative or not your orthodontist is rather than the surgeon. In fact there is some relevance of the surgeon too because he is the one who has a word on the movement needed. The orthodontist if he is a normie one may not be willing to decomp aggressively at all, or he may be experienced but the surgeon may ask for less than the max possible decomp.

Point is you can achieve a class 1 occlusion with different mm movements of BSSO, it's not like there is a single set amount already given by your anatomy.

Really what you want is class 1 molars and class 1 incisors, if you don’t get this then your surgeon/ortho are in fact not conservative but simply incompetent.

And to get this there is only one possible orthodontic treatment and only one possible BSSO movement.

Ignoring things like arch length discrepancy but again if this isn’t resolved, they are incompetent.
