Does a Segmental Lefort 1 increase Intra-Nasal Space?

Im getting jaw surgery soon, and am in consultation w/ the doctor, but they aren’t willing to expand my upper jaw to help treat my complaints re: breathing problems which are in my sinuses and the current plan is only to advance the maxilla, which will help my throat airway, but not my intra-nasal.

So I want to propose to my doctor a different movement which should help the intra-nasal space. So my question is, does a Lefort 1 segmented (split down the middle) increase intra-nasal space? My thinking is that its similar to SARPE because itll increase the palate width, which is connected to base of the septum, hence increasing intra-nasal space. However, because SARPE is not a cut, and it is appliance driven expansion, im unsure if this would also be true for a Lefort 1 segmented; ie, because it is a cut, would expanding the palate of the mouth also mean an increase in the size of the septum floor and intranasal space?

Yes, I got CBCT and full 3D scans. Can I PM you my Google drive ? Ive hosted there. I used the application slicer to look at them, but haven’t done an in depth analysis myself though

What are you movements?
If you’re going to Walline than I suggest that you ask for minimal maxillary advancement bc his cases always have long philtrum post op.

Remember maxilla rotation good
Maxilla advancement (2-4mms) good
Maxilla overadvancment (5+) = chimp lip and ugly philtrum

im getting 8mm of maxilla advancement, but the majority of it is coming from the CCW rotation rather than a forward translation. you think this is fine for me aesthetically? i need this much because my sleep apnea is so fucked

but walline is purported to be the best in the business for sleep apnea which is my main concern, so i guess im sticking with him.

thanks bro. funny thing is i dont even appear that visibly recessed, like i look like a normal human being. my jaw/chin are even more pronounced than andrew tates lol. but my maxilla is pretty recessed visibly and im pretty downswung. the issue is my fucking airway, it feels like im breathing through a straw all the time and honest to god i havent had a night of sleep in like 7 years

I’d still get something like MSE and then a bimax with max 4mms of maxillary rotation.

I’ve worked in the PM alongside many lacoms patients both on here and my old account.

Lacoms, alfaro, both have had many over advanced cases who seeked my help via pms for another round of jaw re-adjustment surgery. So hence why I know.

If you want me to look at your case closer pm me your CT of the skull, a panoramic xray or a proper side profile
