Dr. Eppley has replied:



“With no x-rays and one partial full face I can only make incompletely informed comments:



1) Your lip incompetence is not due to allergies; it is due to a very short chin/lower jaw. At the least a sliding genioplasty of double digits is needed.



2) A long upper lip can only be reduced by a subnasal lip lift. No form of upper jaw shortening/impaction will make the upper lip length any shorter.



3) Orthognathic surgery (bimax surgery) never creates any improvement in the gonial angles and in many cases actually makes it worse. Jaw angle shape changes can only be done with implants.

Idk if he would do Lefort I, I’ll do a maxillary triangle to see how many mms I need for a forward grown maxilla as if that even matters in the first place

he said CCW rotation cannot change ur gonial angle and it will only worsen it and I believe him because I haven’t seen any huge CCW rotations

Why does he refer to the philtrum as upper lip? I have seen other surgeons do it as well and it is so misleading.

Of course it's achievable. Yes he is tilting his head further up in the after photo, but in any bimaxillary surgery where there is maxillary impaction performed, the mandible also has to be moved upwards to close the gap and maintain a proper bite. So the gonial angle decreases.

The decrease is due to the mandible rotating at the TM joint as a response to the maxillary impaction, causing the mandible to "swing" forward, no?

no because a bimaxillary surgery, as the name suggests, includes an osteotomy of the mandible as well. The portion of the mandible that is cut is moved upward and fixed in position with screws/plates
tmj is not involved

correct but the same concept applies with anterior impaction because the fundamental principle is that there is a gap between the surgically displaced maxilla and mandible, so the mandible is also surgically moved to the new position

it doesn't just rotate by itself

I see, also thanks for posting these pics, they helped me visualize something I read a while ago on JSF:
CCW (with posterior downgrafting) also allows for more lower jaw advancement than upper jaw, often with NO extractions involved.

This is patentally wrong bimax with counter clock wise rotation absolutely can fix gonial angle discrepancies also while it's true conventional surgery can't fix the angle of your jaw line you can sculpt your jaw with a jaw sculptor this is done routinely i don't know what eppley is trying to imply here is he saying the girl below didn't improve her gonial angle

Not only that look at her paranasial line it became more upturned her upper midface was untouched that up turn doesn't happen without a rotation and movement no matter angle you use

Does ogee curve really matter for attraction, most people on jaw surgery forums say that implants are shit

Off course it matters the ogee curve means you are low body fat and that you don't have flat cheek bones

Are u serious about Lefort III that is never going to happen. What about modified Lefort III? It advances the orbital rims and cheekbones. Also eppely has a detailed guide on the ogee curve
