Extraction of premolae

Hello, my orthodontist told me that I had to extract premolars because I don't have enough space and I have a trapped canine that hasn't come out. Should I remove the premolars? This is a doubt that has been on my mind.

(I already told my orthodontist about a palate expansion but he told me it wouldn't work in my case)

(I will post my panoramic x-ray so you can help me)

yea bro thats why the doctor said that a regular palate expander won't work.

Unless if you get MSE, which are surgically placed expanders. at 16 your palate suture is definitely fused.
 
When you have a tooth removed, this tooth will take its place, and you'll look exactly the same, only you'll have one less tooth to spare

Don't do anything; it won't affect your health in any way, and vice versa. In the future, if you lose a tooth, this tooth will take its place

Later, there might be devices that can move teeth apart

He told me that it would ruin my face by making my mouth bulge, but my question is if he removes my premolars, will it affect my face or my facial harmony? My doctor also told me leave the wisdom teeth for bone support

If I were you, I'd try to avoid it. If you're fully developed, extracting them will likely have no effect, but it's only camouflaging the problem instead of fixing the main issue, which is that your jaws are too small to accommodate your teeth.

Try to find another ortho who would be open to offering an expander, if you're in the US east coast area, Dr. Zubad Newaz in Philadelphia is a very goated ortho, he's very anti-extraction.
