Rhino : you absolutely need the columella to be grafted. Combined with a lip lift/filtroplasty, it will help to reduce that unsightly philtrum.
Your radix needs to be made taller and wider. Not sure if a regular cartilage/bone graft will be enough ; an implant might be necessary.
The anterior nasal spine could be grafted as well. Would help with nasal projection and philtrum's curve (currently very flat).

Besides a rhino, ptosis repair (eyes), lip lift/filtroplasty, genioplasty (lateral & vertical) are all mandatory for ascension.

Your right eye (left in the pic) might need more than a canthopexy for optimal lower lid support & scleral show.
Based on the pic above, there seems to be a slight strabismus as well. If confirmed, you might want to consult with an ophthalmologist.

Do not neglect softmaxxes : tretinoin/tazarotene for your skin texture, minoxidil + microneedling for closer-set and fuller eyebrows, and leaning out (obviously). If getting leaner doesn't correct your cheeks enough, consider fat dissolvers, lipo, buccal fat removal.

There is a long road ahead of you, but you could end up looking decent if you lock in and if everything goes well.
