They go inside the shoe so you can use them with any shoe. Converse would work as well. Usually they replace the insole

Converse might not work with lifts because they are a pretty tight shoe. i ran in 2 inch lifts in nikes and tripped on my ass ngl

Should be able to enter any shoe. Anything up to 2 inches is fine. I guess you could even go a bit over it if you have a shoe with a high top.
