At your age, people like the Jake in your story seem to be pulled towards other 'cool' kids. For some reason, he might not see you as one but you're probably not the only one. You say you've helped him with X and Y things, do the others do that as well? Think whether or not you doing that makes him see it as a weakness. It is not necessarily a weakness by any means, but people at that age can see it as that. My heart beats very fast under little intensity, & I get chest pains. I have a doctors appointment soon & I'm going to ask my GP for an electrocardiogram & an echocardiogram. It may be something structurally wrong with me, it's not too far fetched that I could have a birth defect or something.
