Getting a bimax for class 3 underbite soon, what should I tell my surgeon?

The surgery will be cosmetic thus I have the opportunity to make suggestions to my surgeon, going on an appointment soon.
Will attach my own profile photos to the thread later, but my general problem is an OVAL face shape and an extremely down grown face which I would like to get fixed after the bimax

Does anybody know anything about it or has any suggestions on what I should ask my surgeon about? Like CCW rotation or whatever contributes to my problem with an oval face shape and down grown face? Would appreciate any advice or suggestion. Something similar to the attached photo but in my case without the severe asymmetry and a more down grown side profile

Something like this but with a more down grown face than the guy on the pic, I bet it has something to do with the recessed maxilla which should be moved forward during the bimax, is there anything I should additionally ask my surgeon about this
