CCW ROTATION
Achieving CCW rotation (counterclockwise rotation of the mandible) can result in a smaller gonial angle, a more square and angular jawline, a more projected chin, and a shorter, more compact midface.
METHOD 1:
Osstivia + hard chewing
After approximately 3–6 months, you may obtain more prominent cheekbones, a wider jaw, and CCW rotation.
METHOD 2:
MARPE
Expanding the palate may provide some forward growth and move the maxilla forward, potentially resulting in CCW rotation.
METHOD 3:
Camouflage with mini-screws
Possible for moderate rotations (usually 2–5°).
10-year follow-up studies show good stability of the results in selected cases. The effect is limited compared with surgery.

Le Fort I + BSSO surgery

- Intrusion of the maxillary molars (and/or the entire dental arch) using TADs / miniscrews:
- Posterior miniscrews (and sometimes anterior ones) allow the upper teeth to be intruded.
- The mandible autorotates counterclockwise (the chin moves upward and forward).
- Used in adults with Class II high-angle, open bite, or excessive vertical growth.
- Total arch intrusion.
- Combined with maximum sagittal anchorage.
