Definitely needs tissue release and fat graft for that mentolabial fold. Thankfully my case isn't as bad as his, so I should be able to ascend with custom implant and / or genio with no need for bimax.

It would lessen the mentolabial fold

Exactly, now he has that pointy witch chin look attached to him. A strong chin doesn't look good when the mandible is this far retruded.
