Hairline lowering, aka forehead reduction, is where they can remove several centimeters of forehead skin(usually 2cm) to lower the hairline. Doing so will give you a lower hairline but still maintain your native hairs, assuming no shock loss. Native hair will always give you a denser look than transplanted hair will. Although you could also lower the hairline several centimeters with a hair transplant, the density will likely be poor given the large amount of coverage and will deplete your donor supply for future hair transplants procedures if hair loss progresses to the crown.

HOWEVER, the big problem with the hairline lowering surgery is the scar it leaves along your hairline. Although the scar is a fine one and usually blends in well with your hair, male hairline recession could make it noticeable in the future. That's obviously an undesirable outcome, however, if you're at worst a Norwood 2/3 and over 30, when future hair loss progression is more predictable than teens and early 20's, and will commit to medical therapies like dutasteride/finasteride/minoxidil, the hairline likely won't recede dramatically enough to reveal the scar for a long time. Even if you continue to lose progress on medication after some time, potentially revealing the scar, the grafts you saved by not lowering your hairline with a transplant can then be allocated to reinforce the hairline.

Scar looks brutal, wonder what it looks like once healed.
fairly certain he's getting a hair transplant to cover it up
healing process + extra hair should do the job
 
wont the skin getting stretched too much be a problem?
 
With verteporfin, it will become even more legit. Especially those who tend to have scars that develop into keloids.

But if verteporfin actually regenerates follicular and not just prevent scarring, I’d rather just transplant the hairline lower.
