Good question. My guess is that he didn't lose any tooth, but just had them rotten and in very bad condition. If you see pics of him smiling, you'll see that he has a very wide palate despite having very bad looking teeth. When he fixed them, his palate became narrower (don't know if he had extractions or the conventional 80's brackets narrowed his palate too).
