A good smile is an indicator of good dento-facial/craniofacial growth. Alot of things have to come together to create a "good smile" its why we see it as aesthetic as its proxy for good health.

Not only that it is an indicator of hygiene, grooming, good surroundings, indicator of good mental wellbeing (also an important trait).

Alot of desirable traits are simply a proxy for good health/genes/development.

My narrow smile came with a narrow jaw and shorter than ideal ramus, it sucks but luckily it's fixable.

Expanding the upper arch is easy but expanding the lower arch is very difficult... The lower arch is a single bone formed from cartilage and not two bones with a suture between them like the upper jaw.

The problem with using an expander in the lower arch is that there is no suture close to the teeth to expand. There are two growth plates in the lower jaw, but they are located up by the joints, not down by the teeth. You can however create a “suture” surgically by cutting the lower jaw into 2 pieces then using an appliance to crank apart the 2 bones (distraction osteogenesis) and then letting it refill and close back into one bone but it doesn't give the most aesthetic results

It's also a very niche surgery, most surgeons don't do MSDO. To quote @RealSurgerymax... Getting a perfect 12 teeth wide palate surgically is not as easy as you think

MSE is a good procedure if you truly need it. It will give very little noticeable difference facially but still a subtle (almost subliminal) improvement in most people, but the biggest reason to have MSE is for a nice wide smile without dark corners.

MSDO is the least aesthetically pleasing of the 3 and IMDO is the most.

An Aesthetic male mandible Is Substantially wider posteriorly than anteriorly (even though a wide chin is masculine the mandibular body should taper inward from the gonial angle connecting to the chin) and MSDO creates the opposite effect in large expansions. That said a small MSDO will not hurt you especially if you truly need to expand the lower arch.

this tbh, i rarely smile and I don't have any wrinkles/ nasolabial folds just in my forehead because I was dumb as a teen and squintmaxxed upwards

studies have shown, that men considered more attractive, when they not smile (also not look aggressive) just have a neutral look

you can google male model and you will see, most of them not smiling.. they just look neutral

but why is that this way? because looking neutral/not smiling = bad boy/masculine/strong thing versus smiling = soft/feminine/women thing

i could search the study that im talking about but only when some people want to read that
(because idk where i have seen that study, so i must search it)

but of course, that just a male thing.. i mean, men will look more attractive, when they not smile.. with the girls are the opposite..

That's such a good point. I've read that post weeks ago, super useful as usual.
I won't benefit from IMDO since I have an already forward grown side profile, my problem is narrow jaw and palate, so it is MSDO with a surgically created suture for me AFAIK, still need to consult this with a surgeon. SARPE+MSDO and then Bimax with ramus widening and jaw angle implants after that should take care of the entire jaw, probably some bone shaving can be done on top of that if the jaw is too U shaped after MSDO. I'm still losing weight before doing more consultations but once I'm lean and do the consultations I'll update here. Surgery is a very, very complex topic, at least for me.

Good thread. I legit have a 6 tooth smile and it looks disgusting af. Hopefully SARPE will ascend me to a 10 tooth smile so I no longer look like a narrow jawed cuck.
Basically the best way to reduce buccal corridors is through SARPE + lefort advancement

I am hoping invisalign improves my teeth show, lefort should help also because my teeth will be closer to the observer rather than set back in my skull because of recessed maxilla

Least attractive doesn't mean unattractive, and in real life smile is even more important because it plays a huge role on how you look in motion. Not everyone wears veneers and yes, there's people born with perfect teeth. Also, there's people that achieved perfect teeth with veneers and or surgery.
