That's just a really terrible angle. Take it properly up at eye height, with even lighting across the face. From that pic alone your eye area actually looks decent. Get your skin cleaned up. If you were psycho you could probably get a chin shield surgery for the fold you have in your chin, but it's probably not necessary.
