Commissuroplasty can increase mouth width but it’s a shit surgery. Unfortunately your eye area flaw (low PFL) also can’t really be fixed.
If I was you I’d just get a lip lift or lip filler, jaw filler to increase jaw width, something that widens the chin and makes it more square (can be done with filler) and also just work out my neck to make it thicker. Your eyebrows could also be thickened. Potentially rhino (your nose could be very slightly slimmer). That’s pretty much all you could do
