How do you determine if you will actually gain ANYTHING from a surgery?

This is often untalked about here and everyone here claims they want a bimax yet many wont gain shit from it.

Does anyone experienced know how to determine:
1. What surgery will ascend you the most points?
2. What surgery will give the most value for money and value for pain endured?

And finally, while assessing individual surgeries, do I even need this one or I'm a retard?

Imagine saving up 10k, travelling, getting the surgery, waiting 6 months - year for results and you still look shit. My biggest fear.
