From a strictly aesthetic point of view, his mandible isn't the problem ; on the other hand, his whole midface looks caved in, set too far back. It's not as severe as syndromic maxillary hypoplasia, but it's bad enough to nerf his side profile (based on that picture).

Midface advancement + lower nasal base :

The whole face now looks much more balanced to me.
