How to prepare for maxfac consultation

I am going to a maxfac mid january, and I need to prepare adequately because I want to convince that nigga that I really need it and insurance should pay for it.

My symptoms:
Ugly asf.

My tongue falls below the mandibular border, and I suspect its because of too little oral volume.

Tmjd on both joints right one more frequently but only a few times a day do I actually pop them. It hurts a decent amount tho

Minor sleep apnea potentially.

Somewhat messed up bite where both upper and lower front teeth are not straight down but rather protrude forwards
