Very few MtFs actually go through with it and even then the important part is testes removal. It's like neutering a dog. Most men should get it actually.
The penis is never lobbed off, it's inverted, but I guess the image makes sense psychologically although most trans paranoia outside it boils down to, “you're just a creep who still wants to dick down women,” “you didn't play with dolls as a kid,” etc., so IMO the messaging’s a bit off here.
