Do you have any backup plan or job that you can work if your plan fails?
Why you dont necessarily need a university degree to have a decent live to pay for surgeries you will need some kind of papers to gain above minimum salery. Cant you go for an easier education path that pays decently, but require more physical labour. Like underwater welder or working in the petrol industry?
That way you can make money at a lower age, perhaps even during internship but in the long run make more money.
