Like if your character is a manlet, people call you short. If you are balding you get weird looks from women NPC.
There would be like a SMV stat that affect how NPC interact with you in general.
Surprised no games do that.

If you play as one race (nosferatu) you have to use the sewers to travel because basically everyone except your own kind is hostile to you on site.
