How come? It’s pretty good for long midfaces and fixes a gummy smile. Obviously don’t get it if you have zero gum show and an ideal fwhr, midface etc, but it’s pretty good

i had posterior downgraft, impaction 1mm at the front of the mouth. you can have lengthening of the face even without a downgraft. le fort 1 forward advancement ITSELF will LENGTHEN the face. you need to impact the jaw upwards A LOT to shorten the face if you are going to advance the upper jaw forward by a good bit.

It will lengthen it yes even without downgrafting because your nose becomes more upturned and upper lip tucks in a bit.

i had it done already, 4mm per side. it's safe if u go to a good surgeon. no relapse or tmj.

How would you go about it if you could do it all over again? Doesn’t downgrafting and impaction also have to take your bite into consideration a.k.a it can’t be done by relying just purely on aesthetics so for example I can’t tell the surgeon to avoid downgrafting because otherwise my occlusion won’t be correct. Or am I wrong?

I wouldnt change a thing the only thing i regret is getting an overadvanced ogre genio. Dont get genio widening. Trust ur surgeon if hes a legit blackpilled one

Okay, I'll avoid downgrafting though as I believe I do not need it. I also have a geniplasty planned, but for me I'm trying to advance my chin only a tad bit just so it's not recessed (its only a tad bit recessed right now) and move it a little bit up, I feel like my chin is a bit too big in comparison to my philtrum, so I'll try to achieve a 2.7 CPR. But I'll defo not let my surgeon overadvance.

He also got Lefort 1, his maxilla was pretty damn recessed thus I assume his movements were not small. With that being said, I don't see any drastic midface length added to his face. His face looks a tad bit, if at all, longer than before.

shortening done of the midface is almost always purely impaction but u cant always impact everyone a lot or else they will end up with SFS and little to no teeth show which looks ugly as fuck.

Absolutely agree, but what I was trying to say that you can see on the pic that she got a LeFort 1, but her face didn't get longer at all.
In the comments someone asked if her maxilla was rotated or if there was something else (for example impaction or downgrafting) done to her and the video creator replied no.

Your maxilla grows down and then they make it less high by cutting some of it away. Absolute scam. The worst result i have ever seen was from a brutal impaction bimax

How brutal is posterior impaction with anterior downgraft? That was my original plan but I insisted on a long face (I had sfs but not particularly bad and no deep bite) so ended up getting global downgraft with cw rotation and no impaction. The downgraft results are shitty and im regretting not getting posterior impaction.

how bad is extractions anyway, if you get it as an adult and need it for decomp? it should have negligible effect compared to something like impaction right?
