My parents are 22-23 years older than me. They are close to paying off the mortgage on the house that's valued at about 500k, I have one other sibling who they'll give money too if they die. If I sell the house and it gets split, I'd be left with about 120k after tax, how long could I sustain this living as frugally as possible? 10-15 years?

Stop wishing your parents bad, they brought you in this world.

And if you moved to SEA with middle class standard of living of 6k a year and average investment returns you would survive on your money indefinitely, in fact it would grow

I'm not wishing them bad, just mentioning they had me young so they may even outlive me since I have shit health due to incel genes (they are both healthy at almost 50 with no problems)

You should invest parts of the money over time

My parents own $500k worth of assets and they are also 25 years older than me. My plan is to surgerymaxx after they're dead but the only difference is I'm also scamming my siblings off their inheritance.

It would be a real tragedy if let's say some unfortunate accident would happen to them...

Put it in a super high interest rate savings account and have yourself a nice 6000 a year supplement to your income
