is there any surgery that would fix a long midface

im planning on getting a rhinoplasty in the future but im worried it wont fix the root problem which is the long midface. so are there any surgeries that would actually fix a long midface

Not super educated on this but I think I heard that the root problem that causes the horse face look is vertical maxillary excess. This makes the length from the eyes to the teeth unusually long, which means that the face needs to elongate somehow to compensate for the extra vertical length. Sometimes that'll be through a longer philtrum, sometimes a longer nose, sometimes a combination of both.

If you have a long philtrum you could in theory fix it with Le Fort 1, as you will literally cut away the bone on which the philtrum rests. You could also do maxillary advancement to make the lip more projected and that way make the philtrum less emphasized. After that you might have to do some soft tissue surgery shit.

If you have a long nose...

Therefore the best way to fix the long face look is by smoke and mirrors: tip plasty to point the nose upwards, make the nose wider with cartilage, do a lip lift if you have a thin upper lip.

thanks alot for this answer my philtrum isnt long I think its definitely the nose and I dont think any surgeon would perform lefort 2 on me for aesthetic reasons and I also heard its insanely risky so idk man ig it just sucks that I have this flaw. I´ll just get the rhino then and potentially a lip lift if the rhino causes my philtrum to appear longer.

Btw here is what orthodontists think about long faces, and you can see what they suggest. It's a lot of the same shit I mentioned but some other things too. The results can be pretty good as you'll see by the pictures.

You can do shit to make it a lot better, even non surgical things or less invasive things like jaw angle implants. You'll solve this shit I trust you bro

thanks man :heart: I got another question tho. would a lefort 1 with ccw rotation not like push the nose up and that way make the midface smaller? or am I wrong

If you could push the nose forwards or get CCW then your nose would look shorter for the same reason your nose looks longer when you look downwards while looking in a mirror, and also why it looks shorter when you look upwards. The maxilla is the same but it's angled in such a way that it looks shorter from a head on view.

But I don't think LF1 would get you this effect. This is because the cut is beneath the nose in LF1, you don't change the bone to which the nose is anchored when you do LF1 (see the img I sent in the first post). But with LF2 you could probably move the nose around.

The midface did become shorter since they mention vertical shortening:

"Orthodontic decompensation in combination with orthognathic and intranasal surgery was chosen. The simultaneous procedures carried out included Le Fort I osteotomy in segments (horizontal advancement, and vertical shortening, counterclockwise rotation, arch expansion, and the correction of the curve of Spee);"

But it's probably minor, I can't honestly put my finger on exactly why she looks more compact. The other girl had a more remarkable result, her nose appears shorter too, probably because of LF1 associated CCWR rather than LF2. Frustrating that they don't mention what they did. There's definitely some lower jaw shit going on, too.

wow her results are good and yeah she does look more compact. it kinda looks like her nasal tip was rotated up but idk what surgeries she got. also I asked ai some stuff.
