Is there any way to fix an underbite without surgery?
 
I was told that headgear doesn’t work as you get older, is ts cope?
Not really. It just works better on younger patients. But Marpe splits the palate and loosens sutures so it can work and give 1-3mm forward growth even till 20 as long as you don't need surgery to split the palate. If you need Sarpe it won't work definetly
 
These depend heavily on age for sure, especially class 3 elastics. They wont do shit skeletally and will just compensate your bite more.

Real question here is how severe of an underbite it is.
Well we dont know. Because no pictures. So we have to assume it's a 14 year old kid or something.

Yea well if he has a bad bite obviously lefort procedures are the only thing saving him.

Yeah, that is sort of true, too many variables play into it. Realistically nothings gonna have a big change on his underbite unless hes under 16 and already in orthodontics
